Credit Dispute Question Cox Communications

I have a question regrading my credit. I found out that some one is using my name and has opened up an account with COX Communication. I want this to be removed from my name in my credit report. How does it work?

I also have another return check charge by Certigy  in my name that was done by someone else but it shows in my credit report. I have been trying to resolve this with certigy but they can’t resolve it since they have no record of me. How can I clear this bad check from credit report?
