Unknown items on my credit report can’t find an address or phone number

I have several “unknown” items on my credit report that state they are medical related but have no contact methods to pay them. I want to pay them since I can get everything negative off of my credit report for less than $400, I just can’t find an address or phone number to do so.
 
Any advice on how I should proceed would be greatly appreciated. Thank you in advance for your help

send it to all thre credit bureaus. Experian. Transunion. Equifax.
 
The account will either fall off
OR they will be confirmed and a letter will be mailed to you with the contact infromation you need.
